La forme de ses interventions doit \u00eatre adapt\u00e9e \u00e0 tous les cas particuliers.<\/strong><\/p>\n\n\n\n<p><strong>En cas de besoin : Dans un futur proche un seul num\u00e9ro d&rsquo;appel t\u00e9l\u00e9phonique permettra de joindre l&rsquo;animatrice de la vie partag\u00e9e qui n\u00e9cessairement se doit d&rsquo;habiter Dieulefit.<\/strong><\/p>\n\n\n\n<p><strong>Parmi les qualit\u00e9s requises: La<\/strong> <strong> r\u00e9activit\u00e9, la discr\u00e9tion, le  respect de l&rsquo;intimit\u00e9 de chacun des demandeurs d&rsquo;aide.<\/strong><\/p>\n\n\n\n<p class=\"has-text-color has-link-color wp-elements-c53900aaa3b5c55b1406aa84305d8f2c\" style=\"color:#c40f91\"><strong>Ses missions pour assurer le bien vivre ensemble, la sant\u00e9, les t\u00e2ches administratives :<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Rencontre et \u00e9coute des habitants.<\/strong><\/li>\n\n\n\n<li><strong>R\u00e9ponse rapide aux urgences.<\/strong><\/li>\n\n\n\n<li><strong>Visites r\u00e9guli\u00e8res aux personnes en difficult\u00e9.<\/strong><\/li>\n\n\n\n<li><strong>Organisation des actions de pr\u00e9vention : vigilance, adaptation des logements, sport personnalis\u00e9.<\/strong><\/li>\n\n\n\n<li><strong>Organisation du \u00ab\u00a0bien manger\u00a0\u00bb pour les personnes en difficult\u00e9.<\/strong><\/li>\n\n\n\n<li><strong>Accompagnement des personnes fragilis\u00e9es pour structurer leur parcours de soins. Structuration avec les professionnels du soin <em>(kin\u00e9s, infirmi\u00e8res)<\/em> et de l&rsquo;aide \u00e0 la personne.&nbsp;&nbsp;&nbsp;&nbsp;<\/strong><\/li>\n\n\n\n<li><strong>Aide \u00e0 la gestion financi\u00e8re des prestations demand\u00e9es et effectu\u00e9es, \u00e0 la charge des b\u00e9n\u00e9ficiaires.<\/strong><\/li>\n\n\n\n<li><strong>Montage des dossiers APA.<\/strong><\/li>\n\n\n\n<li><strong>Travail en lien avec le CCAS, la Mairie, l&rsquo;Agence R\u00e9gionale de Sant\u00e9.<\/strong><\/li>\n\n\n\n<li><strong>Gestion du fichier des adh\u00e9rents compos\u00e9 de \u00ab\u00a0b\u00e9n\u00e9voles aidants\u00a0\u00bb et des personnes \u00e0 aider.<\/strong><\/li>\n\n\n\n<li><strong>Tenue d&rsquo;un cahier de liaison pour toutes les interventions afin d&rsquo;en informer le CA.<\/strong><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>L&rsquo;intervention de personnes qualifi\u00e9es est indispensable \u00e0 notre projet.
